Our office hours are Monday through Friday from 8 a.m. to 4 p.m. Please telephone 212.535.4545 to make an appointment or to discuss your urological concerns.

What to Bring

If you are a new or existing patient, please be sure to bring the following items to your next appointment:

• Identification
• Insurance card
• Referrals (if required by your insurance company)
• X-rays that our physicians have ordered for you
• A list of medications that you are currently taking
• Any co-payments that are required at the time of your visit (if applicable)

Forms

For your convenience, you can print out a patient registration form and a patient history form, in the convenience of your home. Please bring these completed forms with you on your first visit.

Medication Information

The following list is to familiarize you with the medications you may be prescribed. This includes the name of the medication, what it looks like (for easy identification), how to take it, interactions with other drugs, possible side effects, etc. These prescription drugs include:
